IMPACT OF THE IOM REPORT ON NURSING EDUCATION, PRACTICE, AND LEADERSHIP
The demands for health care are increasing each day and will significantly increase with the new Affordable Care Act (ACA) now in place, which was passed by the United States Congress and later signed by President Obama in 2010. The passing of the ACA will ultimately allow millions of people who are un-insured to be insured and receive health care. In 2008, the Robert Wood Johnson Foundation (RWJF) and the Institute of Medicine (IOM) committee spent two years evaluating and transforming the nursing profession in order to help ready the future needs of healthcare. The committee states, “Setting a goal of increasing the percentage to 80 percent (of BSN degree’s) by 2020 is, however, bold, achievable, and necessary to move the nursing workforce to an expanded set of competencies, especially in the domains of community and public health, leadership, systems improvement and change, research, and health policy” ("IOM Nursing Report," 2010). Once I complete my BSN degree, I will be helping the IOM report to meet its goal in attaining 80% of nurses holding a BSN degree by 2020. With me holding a BSN degree, it will open doors for me to be in a leadership role and will allow me to enter a graduate program. A graduate program will then allow me to become an APRN. There is a way to make the BSN percentage goal more achievable and that is by making the BSN program more reasonably priced. There are community colleges which offer a BSN program and it is approximately 20 percent cheaper compared to a university ("IOM Nursing Report," 2010). The downfall to this is there are only a select few community colleges that offer this. If this program were to be offered in more states, then the increase in BSN graduates may increase. “In the community college curriculum, it will be important to teach the important of academic progression and the many educational pathways and opportunities that are available” ("IOM Nursing Report," 2010). This will also help aid in achieving the 80 percent BSN goal by year 2020.
With the ACA in place, nurses will be shifting careers from acute care setting and establishing themselves in a primary care practice setting as an APRN within the community. There are many APRN’s that work under the guidance of a physician, however, their limitation on practice is effected on a state level. Some states allow the APRN the ability to independently prescribe, diagnose, and treat illness and injury whereas in other states, some of these practices are prohibited by an APRN and they must consult a physician to do so. This contraindicates the APRN’s educational background. They were trained to perform these clinical skills, yet are not being allowed to practice to the full extent of their education, which is against the IOM’s key statement. In my state of Arizona, there are no state regulatory requirements for physician involvement in care.
