Military Appearance: SPC Boatwright your daily appearance is substandard on a consistent basis. I expect you to look and act like a leader, however your appearance does not display that you are want that additional responsibility of keep your uniforms and mannerisms to a professional level.

Education: SPC Boatwright I recommend you to enroll in the army correspondence course program to help you with points for promotion and your GT scores also need to improve, I will get you enroll in the Basic Skills Education Program (BSEP) to help you raise your scores.
SPC Boatwright I am not recommending you for promotion at this time because you’re not displaying the type of leadership that an NCO would display, it all starts with your appearance and how professional you are. SPC Boatwright you are an excellent "hard working" soldier but you don't display will and determination to provide a soldier with purpose, direction, and motivation. You will have many opportunities to show improvement and leadership abilities before your next evaluation. You have the potential to be great Soldier; don't mess it up and get on the right track.
You are hereby notified that if this type of conduct continues, actions may be initiated under the Uniform Code of Military Justice to separate you from the Army prior to your scheduled ETS Date IAW AR635200. If you are involuntary separated, you could receive an Honorable Discharge, a General (Under Honorable Conditions) Discharge, or a Under Other Than Honorable Conditions Discharge. If you receive a discharge Under Other Than Honorable Conditions, you will be ineligible for reenlistment and for most benefits, including payments of accrued leave, transitional benefits, the Montgomery GI Bill, VA benefits, and you may also face difficulty in obtaining civilian employment.
