When asked to do a self reflecting account on identifying a new skill achieved during the module, I could have chosen such accomplishments as VAC (Vacuum Assisted Closure) therapy, Maggot therapy or Nutrition and Dietetics. But I wanted to go back to basics and review and fully expand a skill that I knew of, which is Pressure Ulcers (also called pressure sores or bed sores). When I told her that I felt a bit fraudulent by using this reflective account to review a skill I have, in effect, been using for almost two decades. She quickly assured me that in her time as Tissue Viability Nurse she has meet many Nurses, who have been nursing for decades themselves, who still don’t fully understand the management of pressure ulcers and skin viability. She seemed impressed with my courage to meet with her and ask for help. She said that the development of specialised equipment and dressing, to help combat pressure related problems, have changed and improved over the years. I told her that when I was thinking of what skill to reflect on I found a folder on the ward about pressure ulcers and skin care, but I found the folder to be ten years old, and that none of the H.C.A’s or the nurses on the ward had seen the folder. She told me that she tries to get staff updated, but due to time restriction finds the task impossible. I suggested that may be as a project I could collaborate with her and produce a file that the wards and staff could read; the file could be easily readable but be to the point, with diagrams to highlight the point being made. The Nurse also suggested that I go on home visits with her to see some of the patients in the community who have and had problems with pressure ulcers. When I got home I went onto the Wound UK website, recommended by the Tissue Viability Nurse, and was amazed by the amount of information available. From some of the website you could go as in-depth as you wanted, for example how to manage high exudate wounds or how to use negative wound pressure therapy, which I found at first very overwhelming at first but eventfully very use full. This site I felt was set out for Nurses and other medically trained staff, as the contents and terminology was written by other Healthcare Professional for Healthcare Professionals. I looked at another website Your Turn, a campaigning to prevent bed sore, website designed for the general public to visit. This website I felt was very well set out and gave clear and to the point information, it also gave a little test to see if you were at risk. All the sites I looked at provided more and more information than I could have hoped for, they were I thoroughly supported with what they were saying and all seemed to be saying the same things about management and prevention. Each website also provided numerous other links to other sites that I found very useful and interesting. All the sites provided not just well written and had understandable articles, but they had clear pictures and diagrams of where pressure sore are most commonly formed and what they look like in different stages. It was only after reading a leaflet that the Tissue Viability Nurse gave me that I can now identify the difference between a Pressure Ulcer and a Moisture Lesion.
